    Background:

    • Creatine phosphokinase (CPK) is a biomarker typically elevated in muscle diseases.
    • Neuropathic diseases commonly show normal CPK levels.

    Observation:

    • A literature review reveals elevated CPK in 50-75% of motor neuron disease patients, often 5-6 times above normal.
    • Serial CPK measurements in a motor neuron disease patient showed a decrease with bedrest and an increase after moderate exercise.

    Findings:

    • Elevated CPK levels in motor neuron disease appear to be a response to exercise stimuli, not the disease itself.
    • CPK values up to 1000 IU/L are consistent with amyotrophic lateral sclerosis (ALS).

    Implications:

    • Resting patients for 48 hours before testing may yield more accurate CPK readings in cases of suspected motor neuron disease.
