    Summary

    Microsurgery for malignant pineal tumors offers a safe path to complete removal or diagnosis. Early surgical intervention improves outcomes, while radiation alone without tissue diagnosis may be insufficient.

    Background:

    • Primary malignant pineal tumors are rare and challenging to treat.
    • Optimal therapeutic strategies require accurate tissue diagnosis.

    Observation:

    • Six of eight patients with pineal tumors underwent surgical exploration.
    • Complete gross microsurgical excision was achieved in four patients.
    • Two nonoperative patients developed metastatic disease.

    Findings:

    • Surgically treated patients receiving craniospinal radiation showed good outcomes up to 6 years.
    • One patient with pineoblastoma developed spinal metastasis.
    • A patient with germinoma and high human chorionic gonadotropin (HCG) levels responded to chemotherapy.
