    Background:

    • Osteoradionecrosis (ORN) is a serious complication of radiotherapy.
    • Temporal bone osteoradionecrosis can present with persistent otitis externa.
    • Benign mixed tumors of the parotid gland require specific management strategies.

    Observation:

    • This study describes six cases of temporal bone osteoradionecrosis.
    • Patients presented with refractory otitis externa symptoms.
    • The cases highlight the importance of considering ORN in persistent ear infections post-radiation.

    Findings:

    • Conservative local treatments were ineffective for the described cases of ORN.
    • Surgical treatment, including superficial parotidectomy and partial temporal bone resection with facial nerve preservation, was indicated.
    • Benign mixed tumors of the parotid gland were surgically treated, avoiding radiotherapy.

    Implications:

    • Physicians should suspect temporal bone osteoradionecrosis in cases of persistent, treatment-refractory otitis externa.
    • Aggressive surgical management may be necessary for advanced ORN.
    • Radiotherapy should be avoided in the treatment of benign mixed parotid tumors to prevent complications.
