    Summary

Diagnosing deep-seated Candida infections is challenging. This study highlights high mortality rates and identifies surgery as a significant risk factor for systemic candidiasis.

    Area of Science:

    • Medical Mycology
    • Infectious Diseases

    Background:

    • Deep-seated Candida infections pose a serious and growing health concern.
    • Diagnosis of systemic candidiasis is often difficult, leading to delayed or missed treatment.

    Purpose of the Study:

    • To review documented cases of systemic candidiasis at Stanford Medical Center.
    • To assess diagnostic accuracy, mortality rates, and predisposing factors.
    • To evaluate the prevalence of Candida in at-risk patient populations.

    Main Methods:

    • Retrospective review of 40 well-documented cases of systemic candidiasis over 39 months.
    • Analysis of underlying diseases and predisposing factors, including surgery.
    • Study of Candida prevalence in saliva of patients receiving antibiotics or immunosuppressants.
    • Review of serological diagnostic methods for systemic candidiasis.

    Main Results:

    • Diagnosis was suspected premortem in only 47.5% of cases and established in 37.5%.
    • Mortality rate was high at 82.5%, with Candida infection being a primary or major contributing factor in 39.4% of deaths.
    • Surgery was implicated in 50% of cases; higher Candida prevalence was noted in patients on multiple therapies.
    • Precipitin and agglutinin serological methods show promise for diagnosis.

    Conclusions:

    • Systemic candidiasis presents diagnostic challenges and high mortality.
    • Surgery and intensive therapies are significant risk factors.
    • Further research into improved diagnostic methods, particularly serological tests, is warranted.
