    Summary

    Bacterial presence was common in middle ear effusions from chronic otitis media patients. Serous and leukocytic effusions showed higher positive culture rates than mucoid ones, suggesting bacterial roles in otitis media with effusions.

    Background:

    • Chronic otitis media with effusions (OME) is a common condition.
    • The role of bacteria in OME pathogenesis is not fully understood.

    Purpose of the Study:

    • To investigate the bacterial presence in middle ear effusions from patients with OME.
    • To correlate bacterial findings with effusion types.

    Main Methods:

    • Bacterial smear and culture were performed on 175 middle ear effusions from 172 OME patients.
    • Effusions were categorized by type: mucoid, serous, and leukocytic.

    Main Results:

    • Eighty percent of effusions showed positive bacterial smears, while 49% yielded positive bacterial cultures.
    • Positive culture rates varied by effusion type: mucoid (37%), serous (59%), and leukocytic (64%).
    • Common pathogens and nonpathogens each accounted for approximately 50% of isolates.

    Conclusions:

    • Microorganisms are frequently detected in middle ear effusions of OME patients.
    • Bacterial infection may play a significant role in the development and persistence of OME.
    • Effusion type influences bacterial detection rates, with serous and leukocytic effusions being more indicative of bacterial presence.
