Confirmation of gestational age by external physical characteristics (total maturity score)
Insights
The total maturity score (TMS) accurately estimates gestational age in term infants but shows discrepancies in preterm low birthweight babies due to its scoring structure.
Area of Science:
- Neonatal Medicine
- Pediatric Endocrinology
- Maternal-Fetal Medicine
Background:
- Accurate gestational age estimation is crucial for neonatal care and management.
- Existing methods for determining gestational age may have limitations, particularly in specific infant populations.
Purpose of the Study:
- To evaluate the accuracy of the total maturity score (TMS) in estimating gestational age.
- To compare TMS-derived gestational age with established methods in low birthweight infants and controls.
Main Methods:
- Assessed 76 low birthweight infants (<= 2500 g) and 80 matched controls (> 2500 g) with known gestational ages.
- Compared gestational age derived from the last menstrual period with that from the TMS.
Main Results:
- Gestational ages from TMS and last menstrual period were similar in most infants.
- Significant discrepancies were observed in preterm low birthweight infants.
- The scoring system's structure was identified as the cause of discrepancies in preterm infants.
Conclusions:
- The total maturity score (TMS) is a convenient and accurate method for estimating gestational age in term infants.
- TMS may require adjustments or further validation for reliable use in preterm low birthweight populations.
Abstract:
The use of the total maturity score (TMS) as a means of estimating gestational age was assessed in a population of 76 low birthweight infants (less than or equal to 2500 g) and 80 matched controls (greater than 2500 g) all of certain gestational ages. The gestational ages, derived from the last menstrual period and from the TMS, were similar except in preterm low birthweight infants. The discrepancy in this group was attributed to the structure of the scoring system. It is concluded that the TMS is a convenient and accurate method of assessing gestational age in term babies.
