Ultrastructural studies in rheumatoid polyneuropathy.

T Neumark, M Dombay, G Gáspárdy

    Acta Morphologica Academiae Scientiarum Hungaricae
    |January 1, 1979
    PubMed
    Summary

Rheumatoid arthritis patients with neuropathy show nerve damage, including Schwann cell issues and unmyelinated fiber destruction. Electron microscopy revealed vascular changes but no inflammation around nerve vessels.

    Area of Science:

    • Neurology
    • Rheumatology
    • Pathology

    Background:

    • Rheumatoid arthritis (RA) is an autoimmune disease that can affect multiple organs.
    • Neuropathy is a known complication of RA, impacting nerve function.
    • Understanding the specific nerve pathology in RA is crucial for patient management.

    Purpose of the Study:

    • To investigate the ultrastructural changes in the sural nerve of patients with rheumatoid arthritis and neuropathy.
    • To correlate clinical presentation with observed nerve pathology.

    Main Methods:

    • Electron microscopy was used to examine sural nerve biopsy samples from 8 RA patients.
    • Patients presented with sensory and sensomotor neuropathy.

    Main Results:

    • Observed nerve damage included hypermyelination, Schwann cell damage, and destruction of unmyelinated fibers.
    • Proliferation and degeneration of vascular endothelium were noted.
    • No inflammatory signs were detected around peri- and endoneurial vessels.

    Conclusions:

    • Rheumatoid arthritis-associated neuropathy involves significant peripheral nerve damage.
    • Vascular changes in the nerve are present, but inflammation is not a prominent feature.
    • Further research is needed to elucidate the mechanisms driving nerve damage in RA.
