    Summary

    This study found that patients with chronic active hepatitis (CAH) and cholestatic features show more severe bile duct damage in liver biopsies. These histological findings correlate with clinical signs of cholestasis.

    Background:

    • Chronic active hepatitis (CAH) can present with cholestatic features.
    • Bile duct damage is a key pathological finding in liver diseases.

    Purpose of the Study:

    • To investigate the histological characteristics of bile duct epithelium in CAH patients with and without cholestatic features.
    • To correlate histological bile duct damage with clinical and biochemical cholestasis.

    Main Methods:

    • Review of 25 liver biopsies from 12 patients with CAH and cholestasis (Group 1).
    • Review of 30 liver biopsies from 23 patients with CAH without cholestasis (Group 2).
    • Histopathological assessment of bile duct epithelium including hyperplasia, atypia, swelling, vacuolization, and inflammation.

    • Group 1 showed significantly more bile duct epithelial hyperplasia, atypia, swelling, vacuolization, and inflammation compared to Group 2.
    • Histological damage parameters were generally less severe in Group 2.

    • Histological signs of bile duct damage are significantly associated with cholestatic features in CAH.
    • A correlation exists between clinical/biochemical cholestasis and histological bile duct injury in CAH.
