Background:

Sphenoidal sinus aspergillosis is a rare fungal infection.
Oculomotor nerve palsy can be caused by various pathologies, including tumors and infections.

Observation:

A patient presented with oculomotor nerve palsy attributed to sphenoidal sinus aspergillosis.
Initial imaging and clinical presentation suggested a malignant tumor.

Findings:

The sphenoidal sinus aspergillosis was confirmed as the cause of the oculomotor palsy.
Follow-up demonstrated resolution of symptoms, differentiating it from a neoplastic process.

Implications:

This case underscores the importance of considering fungal infections in the differential diagnosis of orbital and neurological symptoms.
