    Background:

    • Emerging evidence suggests peripheral nervous system (PNS) involvement in multiple sclerosis (MS).
    • Histological and electrophysiological studies indicate potential nerve damage outside the central nervous system in MS patients.

    Purpose of the Study:

    • To investigate peripheral nervous system involvement in multiple sclerosis using single-fiber electromyography (SFEMG).
    • To assess nerve conduction and neuromuscular junction function in MS patients.

    Main Methods:

    • Single-fiber electromyography (SFEMG) was performed on 15 patients diagnosed with multiple sclerosis.
    • Analysis focused on jitter measurements, a marker of neuromuscular transmission stability.

    Main Results:

    • Abnormal jitter, indicative of PNS dysfunction, was observed in 10 out of 15 patients.
    • Six patients exhibited clearly abnormal jitter, with two cases revealing previously undiagnosed peripheral neuropathy.
    • Five additional patients showed borderline SFEMG abnormalities.

    Conclusions:

    • The findings provide strong evidence for peripheral nervous system involvement in multiple sclerosis.
    • SFEMG is a valuable tool for detecting subclinical PNS abnormalities in MS.
    • The exact location of the abnormality (terminal nerve network or neuromuscular junction) requires further investigation.
