    Observation:

    • The study reports on a rare case of a cervical thymic cyst.
    • The patient was a 4.5-year-old female with a painless neck swelling.
    • The excised cyst was multilocular, bluish-black, and located on the left side of the neck.

    Findings:

    • Cervical thymic cysts exhibit significant variation in size and fluid color.
    • Histological examination reveals stratified squamous epithelium, Hassal's corpuscles, cholesterol crystals, and lymphocytes.
    • The excised cyst in the reported case displayed all characteristic thymic cyst histological features.

    Implications:

    • Accurate pre-operative diagnosis requires considering thymic remnants in the differential diagnosis of neck masses.
    • Understanding the embryological origin aids in diagnosing these rare cysts.
    • Surgical excision is the primary treatment for symptomatic cervical thymic cysts.
