    Summary

    Severe anorexia nervosa patients may tolerate acetaminophen overdose better than expected. Malnutrition might alter acetaminophen metabolism, preventing liver damage even after a significant overdose.

    Background:

    • Acetaminophen overdose is a common cause of acute liver injury.
    • Malnutrition is often presumed to increase susceptibility to acetaminophen-induced hepatotoxicity.

    Observation:

    • A patient with severe anorexia nervosa ingested 15 gm of acetaminophen.
    • The patient was treated with oral N-acetylcysteine.

    Findings:

    • Despite the large acetaminophen dose and severe malnutrition, the patient showed no signs of liver damage.
    • This contradicts existing literature suggesting increased hepatotoxicity risk in malnourished individuals.

    Implications:

    • Altered acetaminophen metabolism in malnourished patients may confer a protective effect against liver injury.
    • Further research is needed to understand the metabolic changes and their clinical significance in acetaminophen toxicity.
