    Multiple keratoacanthomas can mimic squamous cell carcinoma. Wide surgical excision is recommended for accurate diagnosis and treatment of these skin lesions.

    Background:

    • Keratoacanthomas (KAs) are common skin tumors.
    • Differentiating KAs from well-differentiated squamous cell carcinoma (SCC) can be challenging.

    Observation:

    • A case of multiple keratoacanthomas associated with well-differentiated squamous cell carcinoma is presented.
    • Incomplete excision hinders accurate pathological differentiation between KA and SCC.

    Findings:

    • Multiple KAs can be indistinguishable from SCCs.
    • Conservative treatment of growing lesions may delay management of invasive SCC.

    Implications:

    • Wide surgical excision is the preferred treatment for keratoacanthomas.
    • Prompt diagnosis and treatment are crucial to avoid extensive surgery for invasive SCC.
