    Background:

    • Complete atrioventricular (A-V) block is a serious complication of acute myocardial infarction (MI).
    • The location of the MI can influence the site and severity of the A-V block, impacting patient outcomes.

    Purpose of the Study:

    • To differentiate clinical and electrocardiographic features of complete A-V block in patients with inferior wall MI versus anteroseptal MI.
    • To correlate infarct location with block characteristics, need for pacing, and patient prognosis.

    Main Methods:

    • Analysis of clinical and electrocardiographic records of 20 patients with acute MI and complete A-V block.
    • Categorization of patients based on MI location (inferior wall vs. anteroseptal).
    • Evaluation of block location (above or below bifurcation), presence of Stokes-Adams attacks, need for artificial pacing, and patient outcomes.

    Main Results:

    • Inferior wall MI patients commonly presented with blocks above the bifurcation, transient blocks, no Stokes-Adams attacks, and good outcomes, without requiring artificial pacing.
    • Anteroseptal MI patients frequently had bilateral bundle branch blocks below the bifurcation, severe Stokes-Adams attacks, required artificial pacing, and had a poor prognosis.
    • Although 5 of 7 paced anteroseptal MI patients recovered A-V conduction, all ultimately died.

    Conclusions:

    • Distinct clinical and electrocardiographic patterns of complete A-V block exist in inferior versus anteroseptal MI.
    • Anteroseptal MI with extensive conduction system destruction portends a poor prognosis despite temporary recovery of A-V conduction.
