    Background:

    • Essential mixed cryoglobulinemia (EMC) is a systemic autoimmune disease.
    • Liver involvement is frequently observed in EMC patients, but its characteristics are not fully understood.

    Purpose of the Study:

    • To investigate the prevalence and characteristics of liver disease in patients with EMC.
    • To explore the potential role of hepatitis B virus (HBV) infection in EMC-associated liver disease.
    • To identify distinguishing features of liver disease in EMC.

    Main Methods:

    • Clinical assessment of 30 EMC patients, including liver function tests and physical examination.
    • Liver biopsy in 12 patients to evaluate histological changes.
    • Serological testing for hepatitis B surface antigen (HBsAg) and antibody (HBsAb).
    • Analysis of complement profiles (CH50, C4, C3PA).

    Main Results:

    • Twenty-one out of 30 (70%) EMC patients showed evidence of liver involvement.
    • Liver disease was often asymptomatic, with mild enzyme elevations and abnormal retention of BSP.
    • Histological findings included chronic persistent hepatitis, chronic active hepatitis, or cirrhosis.
    • Hepatitis B virus markers were found in 44% of patients, but liver lesions were similar across HBV-exposed and unexposed groups.
    • EMC patients exhibited a characteristic complement profile (low CH50 and C4, normal C3PA).

    Conclusions:

    • Liver disease is a common manifestation of EMC, potentially representing a distinct entity termed 'cryoglobulinemic hepatitis'.
    • HBV infection is frequent in EMC but may not be the sole determinant of liver pathology.
    • Specific complement profiles can aid in differentiating EMC-related liver disease from other chronic liver conditions.
