    Dementia affects nearly a third of Parkinson's disease (PD) patients, occurring more frequently than in controls. PD with dementia may be a distinct condition from PD without cognitive impairment.

    Background:

    • Parkinson's disease (PD) is a progressive neurodegenerative disorder.
    • Cognitive impairment, including dementia, is a known comorbidity in PD.
    • The relationship between PD and dementia requires further elucidation.

    Purpose of the Study:

    • To determine the incidence and characteristics of dementia in a cohort of patients with parkinsonism.
    • To compare demographic and clinical features between PD patients with and without dementia.
    • To investigate the potential distinction between PD with dementia and PD without dementia.

    Main Methods:

    • Retrospective analysis of 520 patients diagnosed with parkinsonism over eight years.
    • Assessment of dementia severity (moderate to marked).
    • Comparison of age, PD onset, disease severity, and levodopa response between demented and non-demented groups, including age-matched controls.

    Main Results:

    • Dementia was observed in 168 (32%) of the 520 patients.
    • Demented patients were older (70.4 vs. 65.5 years) and developed PD later.
    • Dementia incidence in PD was tenfold higher than in controls; demented patients showed faster progression and poorer levodopa response.

    Conclusions:

    • Dementia is a significant comorbidity in Parkinson's disease, occurring at a substantially higher rate than in the general population.
    • Patients with PD and dementia exhibit distinct clinical characteristics, including later onset, more rapid progression, and reduced levodopa efficacy.
    • PD with dementia may represent a distinct clinicopathological entity separate from PD without dementia.
