    Summary

    Patients with sarcoidosis exhibit lymphocyte sensitization to antigens like tuberculin, despite negative skin tests. This immune anomaly was also observed in two BCG-inoculated patients who failed to convert, with no clear explanation.

    Background:

    • Sarcoidosis is an inflammatory disease characterized by granuloma formation.
    • Immune responses in sarcoidosis patients often present atypical features.
    • Standard delayed-type hypersensitivity skin tests may be unreliable in sarcoidosis.

    Purpose of the Study:

    • To investigate lymphocyte sensitization patterns in sarcoidosis patients.
    • To explore the immune response to specific antigens in sarcoidosis.
    • To examine cell sensitization in patients unresponsive to BCG vaccination.

    Main Methods:

    • Blood samples from 19 sarcoidosis patients were analyzed.
    • Lymphocyte sensitization to purified protein derivative of tuberculin (PPD) and Kveim agent was assessed.
    • Cellular immune responses were evaluated in two patients with failed BCG conversion.

    Main Results:

    • All 19 sarcoidosis patients demonstrated lymphocyte sensitization to PPD and Kveim agent.
    • Despite antigen sensitization, patients exhibited negative or significantly reduced Mantoux reactions.
    • Two BCG-inoculated patients, who failed to convert, showed similar cell sensitization phenomena.

    Conclusions:

    • Sarcoidosis is associated with a distinct pattern of lymphocyte sensitization.
    • A discrepancy exists between in vitro cell sensitization and in vivo skin test reactivity in sarcoidosis.
    • The observed immune anomaly in sarcoidosis warrants further investigation.
