    Background:

    • Capillary permeability is crucial in various physiological and pathological processes.
    • Assessing generalized capillary permeability changes is clinically significant for diagnosing and managing diseases.
    • Existing methods may not accurately reflect dynamic changes in capillary permeability.

    Purpose of the Study:

    • To develop and validate a method for quantifying generalized capillary permeability.
    • To investigate capillary permeability in patients with systemic lupus erythematosus, idiopathic thrombocytopenic purpura, chronic active hepatitis, liver cirrhosis, and subacute bacterial endocarditis.
    • To explore the relationship between capillary permeability and serum factors.

    Main Methods:

    • Simultaneous injection of 131I-HSA (escapes vascular beds) and 51Cr-labeled red blood cells (confined to blood).
    • Determination of total plasma volume using both tracers.
    • Calculation of the difference (deltaTPV) between plasma volumes to assess capillary permeability increase.

    Main Results:

    • Patients with systemic lupus erythematosus, idiopathic thrombocytopenic purpura, chronic active hepatitis, liver cirrhosis, and subacute bacterial endocarditis showed significantly increased deltaTPV compared to controls.
    • Average deltaTPV values were 204ml/m2, 178 ml/m2, 82 ml/m2, 131 ml/m2, and 179 ml/m2, respectively.
    • A serum capillary permeability increasing factor was detected in patients with elevated deltaTPV, with a significant correlation between deltaTPV and the factor's titer.

    Conclusions:

    • The deltaTPV method effectively quantifies generalized capillary permeability.
    • Increased capillary permeability is a feature of systemic lupus erythematosus, idiopathic thrombocytopenic purpura, chronic active hepatitis, liver cirrhosis, and subacute bacterial endocarditis.
    • A serum factor likely contributes to increased capillary permeability in these conditions.
