    Summary

    This study examines four Helminthosporium corneal ulcer cases. Despite good in vitro antifungal sensitivity, clinical responses varied, suggesting complex treatment challenges for fungal keratitis.

    Background:

    • Helminthosporium species are rare but serious causes of fungal keratitis.
    • Corneal ulcers require prompt diagnosis and appropriate antifungal therapy.

    Observation:

    • Four distinct cases of Helminthosporium corneal ulcers were analyzed.
    • Patient 1: Immune-deficient infant.
    • Patient 2: Post-foreign body removal.
    • Patient 3: Corneal dystrophy, soft contact lens wear, topical corticosteroids.
    • Patient 4: History of multiple foreign bodies.

    Findings:

    • Antifungal sensitivities were tested in three cases, showing good in vitro susceptibility to available medications.
    • All four patients demonstrated clinical improvement.
    • A poor correlation was observed between in vitro antifungal sensitivities and actual clinical outcomes.

    Implications:

    • This highlights the challenges in treating fungal keratitis caused by Helminthosporium.
    • Clinical response may not always align with laboratory antifungal susceptibility testing.
    • Further research is needed to optimize treatment strategies for complex fungal corneal ulcers.
