    Men who had orchiopexy for undescended testis showed abnormal spermatogenesis. This suggests potential bilateral testicular issues even with unilateral undescended testis.

    Background:

    • Undescended testis (cryptorchidism) is a common congenital anomaly.
    • Orchiopexy is a surgical procedure to correct undescended testis.
    • Long-term testicular function after orchiopexy requires further investigation.

    Purpose of the Study:

    • To assess testicular function in adult men who underwent orchiopexy for unilateral undescended testis.
    • To compare hormonal profiles and sperm parameters between patients and controls.
    • To investigate potential bilateral testicular abnormalities.

    Main Methods:

    • Study included 29 men (age 21-35) post-orchiopexy for unilateral undescended testis.
    • Hormonal assays: serum testosterone, dialyzable testosterone, basal and GnRH-stimulated LH.
    • Semen analysis: sperm density.
    • Comparison with a control group of 30 normal men.

    Main Results:

    • Serum testosterone and LH levels were comparable to controls.
    • Sperm density was significantly lower (one-third) in patients compared to controls (p < 0.001).
    • Follicle-stimulating hormone (FSH) response to GnRH was significantly elevated in patients (p < 0.001).

    Conclusions:

    • Orchiopexy for unilateral undescended testis may be associated with impaired spermatogenesis.
    • Elevated FSH suggests a potential primary testicular defect.
    • Findings indicate a possible bilateral testicular abnormality in men with a history of unilateral undescended testis.
