    • A case of DORV presented with progressive muscular subvalvular aortic stenosis.
    • Systolic pressures within the ventricle exceeded aortic pressure, indicating a significant gradient.
    • Angiocardiography revealed a hypertrophied subaortic conus obstructing the right ventricular outflow tract.

    Findings:

    • The subaortic stenosis was unrelated to the ventricular septal defect.
    • Serial studies demonstrated progression of the stenosis.
    • Pulmonary artery banding, performed earlier, was a potential contributing factor to the stenosis progression.

    Implications:

    • Distal conal hypertrophy is proposed as the mechanism for obstruction in this DORV case.
    • This finding highlights a potential complication of pulmonary artery banding in DORV patients.
    • Further research into the development of conal hypertrophy in congenital heart disease is warranted.
