    Associative visual agnosia resulted from bilateral occipitotemporal lesions. Intact visual-limbic connections and spared corpus callosum may explain the absence of alexia in this case.

    Background:

    • Associative visual agnosia is a condition where patients can see objects but cannot recognize them.
    • The underlying neural mechanisms and specific lesion correlates are not fully understood.

    Observation:

    • A patient presented with associative visual agnosia but no alexia.
    • Neuroimaging revealed bilateral infarction in the posterior cerebral arteries affecting occipitotemporal regions.

    Findings:

    • The patient had corticosubcortical lesions in both occipitotemporal regions, sparing the corpus callosum.
    • Bilateral lesions of the inferior longitudinal fasciculi were identified as potential necessary and sufficient lesions for this syndrome.
    • The intact corpus callosum might have prevented the development of alexia.

    Implications:

    • Bilateral loss of visual-limbic connections is implicated in associative visual agnosia.
    • Understanding specific lesion patterns can differentiate between visual agnosia and alexia.
    • This case highlights the critical role of specific white matter tracts in visual recognition and reading.
