    Summary

    Carbidopa-levodopa (Sinemet) therapy in Parkinson disease patients does not significantly alter thyroid function. While a minor reduction in thyrotropin (TSH) was observed, overall thyroid hormone levels remained stable in euthyroid individuals.

    Background:

    • Carbidopa-levodopa (Sinemet) is a primary treatment for Parkinson disease.
    • Potential interactions between Sinemet and thyroid function require investigation.

    Purpose of the Study:

    • To assess the impact of chronic carbidopa-levodopa therapy on thyroid function tests in euthyroid Parkinson disease patients.

    Main Methods:

    • Thyroid function was evaluated in patients on Sinemet and controls.
    • Tests included serum thyroxine (T4), triiodothyronine (t3), thyrotropin (TSH), T3 uptake (T3U), free T4 index (FT4I), and free T3 index (FT3I).
    • Measurements were taken in fasting states and 2 hours post-drug administration.

    Main Results:

    • All subjects tested as euthyroid.
    • No significant differences in thyroid hormone levels were found between patients and controls.
    • A small but significant reduction in serum TSH levels was noted after Sinemet administration.

    Conclusions:

    • Therapeutic doses of carbidopa-levodopa do not significantly affect overall thyroid function in euthyroid Parkinson disease patients.
    • The observed TSH reduction warrants further investigation but does not indicate clinical hypothyroidism.
