The "hypernormal" CT scan in dementia: bilateral isodense subdural hematomas.

P L Jacobson, T W Farmer

    Neurology
    |November 1, 1979

    Summary

Hypernormal computed tomography (CT) scans in elderly dementia patients may indicate bilateral isodense subdural hematomas. Further investigation with isotope brain scans or angiography is recommended for accurate diagnosis.

    Area of Science:

    • Neurology
    • Radiology
    • Geriatrics

    Background:

    • Computed tomography (CT) scans commonly reveal enlarged ventricles and prominent sulci in elderly patients with dementia.
    • These imaging findings are often associated with cognitive decline and neurodegenerative processes.

    Observation:

    • Three elderly demented patients presented with "hypernormal" CT scans, characterized by small, symmetric ventricles and minimal sulci.
    • These findings were atypical given the patients' clinical presentation and age.

    Findings:

    • Subsequent diagnostic evaluations, including isotope brain scans and angiography, revealed bilateral isodense subdural hematomas in all three patients.
    • This suggests that subdural hematomas can present with unexpectedly normal-appearing CT scans.

    Related Experiment Videos

    Implications:

    • A "hypernormal" CT scan in an elderly demented patient warrants high suspicion for bilateral isodense subdural hematomas.
    • Further diagnostic imaging, such as isotope brain scans or angiography, is crucial for confirming the diagnosis and guiding treatment.
