    Summary

    Intramedullary cysticercosis, a rare spinal cord infection, presented as low back pain and leg weakness. This case highlights the first instance of spinal cysticercosis diagnosed in Japan.

    Background:

    • Intramedullary cysticercosis is an exceptionally rare parasitic infection of the spinal cord.
    • Only 26 cases have been documented globally prior to this report.
    • Spinal cysticercosis can mimic other neurological conditions, complicating diagnosis.

    Observation:

    • A patient presented with low back pain, paraparesis, and lumbosacral sensory loss.
    • Signs of meningeal irritation were also noted.
    • Initial clinical diagnosis suggested a spinal cord tumor.

    Findings:

    • Surgical exploration revealed a cysticercus cyst within the lumbosacral spinal cord.
    • No evidence of parasitic infection was found elsewhere in the body.
    • This represents the first reported case of spinal cysticercosis in Japan.

    Implications:

    • This case expands the known geographical distribution of spinal cysticercosis.
    • It underscores the importance of considering parasitic infections in the differential diagnosis of spinal cord lesions.
    • Further awareness and diagnostic vigilance are crucial for early detection and treatment of this rare condition.
