    Summary

    Patients with progressive supranuclear palsy (PSP) and suspected normal-pressure hydrocephalus (NPH) showed temporary gait and cognitive improvements after shunt surgery. However, key PSP symptoms like gaze paralysis remained unaffected.

    Background:

    • Progressive supranuclear palsy (PSP) is a rare neurodegenerative disease.
    • Normal-pressure hydrocephalus (NPH) is a treatable condition characterized by gait disturbance, cognitive decline, and urinary incontinence.

    Observation:

    • Three patients presented with clinical features of PSP.
    • Radiological imaging in these patients suggested NPH.

    Findings:

    • Shunt placement in these patients led to transient improvements in gait, mentation, and bladder control.
    • Gaze paralysis and other extrapyramidal signs, characteristic of PSP, did not improve post-surgery.

    Implications:

    • This case series highlights the diagnostic challenge in differentiating PSP from NPH.
    • It suggests that while shunting may alleviate some hydrocephalus-related symptoms, it does not alter the underlying progression of PSP.
