Laryngeal coccidioidomycosis.

M A Platt

    JAMA

    Summary

Severe pulmonary coccidioidomycosis can rarely affect the larynx. This case highlights a potential direct inoculation complication of Coccidioides immitis infection.

    Area of Science:

    • Medical Mycology
    • Infectious Diseases
    • Pulmonology

    Background:

    • Coccidioidomycosis is a fungal infection endemic to the southwestern United States.
    • Pulmonary involvement is the most common presentation of Coccidioidomycosis.
    • Laryngeal involvement is an exceptionally rare manifestation.

    Observation:

    • A patient presented with severe bilateral pulmonary coccidioidomycosis.
    • Laryngeal involvement was diagnosed via biopsy.
    • The causative agent was identified as Coccidioides immitis.

    Findings:

    • This case represents a rare instance of laryngeal involvement secondary to pulmonary Coccidioidomycosis.
    • The presumed mechanism of laryngeal involvement is direct inoculation.
    • Such complications have been documented infrequently in medical literature.

    Implications:

    • Highlights the importance of considering unusual presentations in immunocompromised or severely ill patients.
    • Suggests the need for vigilance in diagnosing and managing disseminated or complicated coccidioidomycosis.
    • Further research may elucidate the precise mechanisms and risk factors for direct inoculation in laryngeal coccidioidomycosis.
