    Area of Science:

    • Oncology
    • Surgical Oncology
    • Palliative Care

    Background:

    • Advanced pelvic malignancies often present with debilitating symptoms.
    • Previous treatments, including radiotherapy, may fail, necessitating radical surgical options.

    Purpose of the Study:

    • To evaluate the safety and efficacy of pelvic exenteration in managing advanced pelvic malignancies.
    • To assess the impact of pelvic exenteration on symptom palliation and quality of life.

    Main Methods:

    • Retrospective review of 18 patients who underwent pelvic exenteration.
    • Analysis of postoperative morbidity, mortality, and long-term symptom control.

    Main Results:

    • One postoperative death occurred in 18 patients.
    • Minimal morbidity was observed, even in elderly patients with prior radiotherapy failure.
    • Significant symptom-free periods were achieved for patients with pelvic pain, fistulas, sepsis, hemorrhage, and incontinence.

    Conclusions:

    • Pelvic exenteration provides substantial symptomatic palliation for advanced pelvic malignancies.
    • The procedure can be safely performed with acceptable morbidity and mortality.
    • A liberal approach to pelvic exenteration is warranted for selected patients, focusing on quality of life when cure is not anticipated.
