    Background:

    • Graves' disease is an autoimmune disorder causing hyperthyroidism.
    • Thionamide drugs are used to manage hyperthyroidism by reducing thyroid hormone synthesis.
    • Understanding the molecular mechanisms of thyroid hormone production in Graves' disease is crucial for effective treatment.

    Purpose of the Study:

    • To compare in vitro iodination of thyroglobulin in toxic diffuse goiter (Graves' disease) versus normal thyroid tissue.
    • To investigate the effect of triiodothyronine (T3) administration on thyroid iodination in Graves' disease patients.
    • To determine the role of peroxidase activity in the observed differences in iodination.

    Main Methods:

    • Thyroid tissue samples were obtained from patients with Graves' disease (treated and untreated with T3) and euthyroid controls.
    • Thyroglobulin and particulate fractions were isolated using sucrose density gradient centrifugation and digitonin treatment.
    • In vitro iodination assays were performed using 125I-, glucose, and glucose oxidase.
    • Peroxidase activity was assessed by guaiacol oxidation.

    Main Results:

    • Iodine binding to thyroglobulin was significantly higher in toxic diffuse goiter tissue compared to normal thyroid tissue.
    • Triiodothyronine (T3) administration did not alter the increased iodination in Graves' disease patients.
    • The enhanced iodination in Graves' disease was attributed to increased peroxidase activity in the particulate fraction.

    Conclusions:

    • In vitro iodination of thyroglobulin is elevated in toxic diffuse goiter, irrespective of thionamide treatment or T3 administration.
    • The increased iodination is primarily due to enhanced peroxidase activity within the particulate fraction of thyroid tissue.
    • These findings highlight a persistent molecular abnormality in thyroid hormone synthesis in Graves' disease.
