    Summary

    Extensive diagnostic evaluations for childhood hypertension may not always be necessary. Some children with essential hypertension can become normotensive without intervention, especially if target-organ involvement is absent.

    Background:

    • Hypertension in children is a growing concern.
    • Essential hypertension, without a clear secondary cause, is increasingly diagnosed in pediatric populations.
    • Defining the necessity of extensive diagnostic workups for pediatric hypertension is crucial.

    Purpose of the Study:

    • To evaluate the outcomes of children diagnosed with hypertension.
    • To determine if extensive diagnostic evaluations are always required for pediatric hypertension.
    • To assess the spontaneous resolution rate and treatment efficacy in hypertensive children.

    Main Methods:

    • A cohort of 15 pediatric patients (aged 8-17 years) with hypertension (supine diastolic blood pressure >90 mm Hg) was studied.
    • All patients underwent extensive diagnostic evaluations to exclude secondary causes.
    • Treatment with hydrochlorothiazide was administered to patients with target-organ involvement; others were untreated.

    Main Results:

    • Extensive diagnostic evaluations failed to identify an underlying cause in all cases, suggesting essential hypertension.
    • Five of the treated patients with target-organ involvement became normotensive.
    • Four of nine untreated patients spontaneously normalized blood pressure within 8-14 months.

    Conclusions:

    • Extensive diagnostic studies may not be essential for all children diagnosed with essential hypertension.
    • Close monitoring and selective intervention are warranted, particularly in cases without target-organ involvement.
    • Spontaneous resolution of hypertension can occur in a subset of pediatric patients.
