    Summary

    Alpha-antitrypsin (AAT) deposition in liver tissue is associated with primary liver cancer. Immunohistochemistry is more sensitive than PAS staining for detecting AAT in tumor cells.

    Background:

    Purpose of the Study:

    • To investigate the presence and significance of alpha-antitrypsin (AAT) deposition in patients with primary hepatic carcinoma.
    • To compare the sensitivity of immunoperoxidase staining with the PAS reaction for detecting AAT in hepatic tissues.

    Main Methods:

    • Paraffin-embedded hepatic tissue samples from 21 patients with primary hepatic carcinoma were analyzed.
    • Immunoperoxidase staining was used to detect alpha-antitrypsin (AAT) deposition.
    • PAS reaction was employed for comparison of sensitivity.

    Main Results:

    • Specific AAT immunoreactivity was detected in tumor cells of 50% of cases, sometimes co-occurring with positive nonneoplastic hepatocytes.
    • Immunohistochemistry demonstrated higher sensitivity for AAT detection compared to PAS staining.
    • Three types of cytoplasmic tumor cell inclusions were observed, with solid inclusions identified as immunoreactive AAT deposits.

    Conclusions:

    • The study confirms an association between hepatic AAT deposition and the occurrence of primary liver carcinoma.
    • Further prospective studies involving serum and tissue analysis in hepatoma patients are recommended to validate these findings.
