    Summary

    The propranolol-glucagon test reliably stimulates human growth hormone (HGH) secretion in children. This test is a sensitive diagnostic tool for assessing HGH levels, even in patients with non-pituitary conditions.

    Background:

    • Assessing human growth hormone (HGH) secretion is crucial for diagnosing growth disorders in children.
    • Existing diagnostic methods may have limitations in sensitivity and reliability.

    Purpose of the Study:

    • To evaluate the efficacy and reliability of a combined propranolol-glucagon test for stimulating HGH secretion.
    • To determine optimal dosages for glucagon and propranolol in pediatric testing.

    Main Methods:

    • A propranolol-glucagon stimulation test was administered to 24 healthy children, 21 with pituitary dwarfism, and others with various conditions.
    • Standardized doses of glucagon (30 µg/kg) and propranolol (0.2–1.0 mg/kg) were used.
    • Serum HGH, 11-OHCS, and urinary catecholamines were measured.

    Main Results:

    • The test significantly increased serum HGH levels in control subjects (from 2.3 to 30.0 ng/ml).
    • Serum 11-OHCS levels also showed a significant increase (P < 0.01).
    • No significant difference in catecholamine levels was observed between pituitary dwarfs and controls.

    Conclusions:

    • Propranolol-glucagon administration is a sensitive and reliable provocative test for HGH secretion.
    • False negative HGH responses were not observed in patients with non-pituitary diseases, suggesting broad applicability.
    • The exact mechanism of HGH response to this combined test requires further investigation.
