    Background:

    • Enlarged thyroid (goiter) can cause tracheal compression, but its incidence in euthyroid goiter is not well-established.
    • Patients with nontoxic goiter may not report respiratory symptoms like dyspnea, despite potential airway compromise.

    Purpose of the Study:

    • To investigate the prevalence of upper airway obstruction in patients with nontoxic goiter.
    • To assess the impact of goiter therapy on upper airway function.

    Main Methods:

    • Twenty patients with nontoxic goiter underwent evaluation for upper airway obstruction using roentgenography and flow volume loops (FVL).
    • Patients were questioned about exertional shortness of breath and choking sensations.
    • FVL parameters were reassessed after goiter treatment.

    Main Results:

    • While most patients (9/20) reported subtle symptoms, objective tests revealed significant obstruction: 35% by roentgenography, 60% by FVL, and 80% by combined methods.
    • Goiter therapy led to reduced goiter size and improved FVL.
    • Statistically significant improvements were observed in peak expiratory flow (P < .01) and peak inspiratory flow (P < .05), indicating reduced airway compression.

    Conclusions:

    • Nontoxic goiter commonly causes unsuspected upper airway obstruction, often developing insidiously.
    • Treatment of goiter can alleviate airway compression and improve respiratory function.
