    Summary

    This study examined liver biopsies from secondary syphilis patients, finding no specific ultrastructural changes. Electron microscopy did not detect the syphilis-causing bacteria, Treponema pallidum, in liver tissue.

    Background:

    • Secondary syphilis can affect multiple organs, including the liver.
    • Liver involvement in syphilis may present with non-specific histological findings.
    • Previous studies have limited ultrastructural data on hepatic involvement in secondary syphilis.

    Purpose of the Study:

    • To investigate the ultrastructural liver biopsy findings in patients with secondary syphilis.
    • To determine if specific cellular or ultrastructural changes are associated with hepatic syphilis.
    • To correlate light microscopy findings with electron microscopy observations.

    Main Methods:

    • Examination of liver biopsy specimens from 19 patients with secondary syphilis.
    • Patients were unselected, hepatitis B surface antigen-negative, and non-jaundiced.
    • Ultrastructural analysis using electron microscopy was performed.

    Main Results:

    • Most hepatocytes appeared normal, with some showing minor, non-specific changes like lipofuscin accumulation.
    • Kupffer cell and endothelial cell hyperplasia were observed in some cases, sometimes with associated mononuclear cells.
    • Treponemes were visualized by light microscopy in three cases, but not detected via electron microscopy in the selected blocks.

    Conclusions:

    • Secondary syphilis does not appear to cause specific ultrastructural changes in the liver.
    • The observed non-specific findings, such as Kupffer cell hyperplasia, lack diagnostic value for syphilis.
    • Electron microscopy was unable to confirm the presence of treponemes within the liver tissue examined.
