    Summary

    Isolated leptomeningeal myeloma, a rare central nervous system (CNS) cancer, involves abnormal proteins produced directly within the cerebrospinal fluid (CSF). This case highlights in situ protein production by myeloma cells in the leptomeninges.

    Background:

    • Multiple myeloma frequently causes neurological symptoms due to cranial or vertebral involvement.
    • Central nervous system (CNS) or meningeal invasion by myeloma is uncommon.
    • Intracranial or intraspinal myeloma without adjacent bone lesions is exceptionally rare.

    Observation:

    • This report details the third case of isolated leptomeningeal myeloma.
    • Previous literature documents only ten cases of isolated dural or brain myeloma, with two limited to the leptomeninges.
    • The current case presented with myeloma cells and abnormal globulins exclusively in the cerebrospinal fluid (CSF), with serum and bone marrow remaining unaffected.

    Findings:

    • Analysis of 13 reported cases of dural, brain, or leptomeningeal myeloma supports a specific pathogenetic concept.
    • Abnormal proteins in the CSF are not derived from serum but are synthesized locally.
    • Myeloma cells within the CNS or meninges produce abnormal proteins in situ.

    Implications:

    • This understanding refines diagnostic approaches for neurological complications in multiple myeloma.
    • It suggests targeted therapies may be beneficial for CNS or leptomeningeal myeloma.
    • Further research into the mechanisms of CNS invasion and local protein production is warranted.
