    • Clinical improvement in motor function occurred, but sensory deficits persisted.

    Findings:

    • Autopsy revealed a lacunar infarct in the ventral posterior nucleus of the left thalamus.
    • Microscopic examination showed extension of the infarct into the adjacent posterior limb of the internal capsule.
    • This thalamocapsular infarct was confirmed as the cause of the patient's sensorimotor stroke.

    Implications:

    • This case presents a unique clinico-pathological correlation of a sensorimotor stroke.
    • It underscores the role of the thalamus and internal capsule in integrated sensorimotor processing.
