    Background:

    • A newborn presented with macrocephaly and severe anemia following an elective cesarean section.
    • Initial management included exchange transfusion for anemia, with apparent initial stability.

    Observation:

    • The infant subsequently developed poor feeding and seizures, despite initial stabilization.
    • Skull transillumination was negative; however, a tense anterior fontanel was noted.
    • Computerized tomography revealed mild hydrocephalus and suggested subdural fluid collection.

    Findings:

    • Bilateral subdural taps confirmed chronic subdural hematoma.
    • This represents the first reported case of intrauterine subdural hematoma in the absence of physical trauma.
    • Pathogenesis may be analogous to subdural hematoma formation after rapid cerebrospinal fluid decompression.

    • Highlights a rare, non-traumatic cause of intrauterine subdural hematoma in neonates.
    • Suggests potential mechanisms related to rapid fluid shifts or pressure changes in utero.
