    Background:

    • D-penicillamine is a chelating agent used to treat various conditions, including arthritis.
    • Drug-induced pemphigus is a known but uncommon side effect of D-penicillamine therapy.
    • Pemphigus is a group of autoimmune blistering diseases affecting the skin and mucous membranes.

    Observation:

    • A patient with arthritis developed a dermatitis herpetiformis-like rash during D-penicillamine treatment.
    • The rash resolved after discontinuation of the medication.
    • Histopathological examination revealed intra-epidermal vesicles and spongiosis, consistent with pemphigus but without acantholysis.

    Findings:

    • Indirect immunofluorescence detected intercellular antibodies at the time of the rash and in pre-treatment serum.
    • The presence of antibodies before D-penicillamine treatment suggests a pre-existing susceptibility.
    • This case highlights the potential for D-penicillamine to unmask or trigger pemphigus in predisposed individuals.

    Implications:

    • Clinicians should consider drug-induced pemphigus in patients developing new rashes during D-penicillamine therapy.
    • Pre-treatment screening for autoantibodies might be considered in certain patient populations.
    • Understanding predisposition factors can aid in managing and preventing adverse drug reactions.
