    Summary

    Distinguishing diffuse epithelial mesothelioma from adenocarcinoma can be challenging. This study identifies key ultrastructural and cytologic features of poorly differentiated mesothelioma, aiding accurate diagnosis in pleural and peritoneal biopsies.

    Background:

    • Accurate differentiation between diffuse epithelial mesothelioma and metastatic adenocarcinoma is crucial for patient management.
    • Traditional diagnostic criteria rely on microvilli and tonofilaments, but poorly differentiated tumors present challenges.

    Observation:

    • This study examined 5 cases of diffuse epithelial mesothelioma with significantly altered fine structural features.
    • Tumor cells in these cases often showed poor differentiation, lacking typical intermediate filaments, desmosomes, and microvilli.

    Findings:

    • Immunohistochemistry confirmed cytokeratin presence and carcinoembryonic antigen absence in all cases.
    • Distinct cytologic features, including a mosaic cell pattern and specific nuclear characteristics, were identified for poorly differentiated mesothelioma.
