    Background:

    • Nephrotic syndrome is a complex kidney disorder.

    Purpose of the Study:

    • To investigate serum immunoglobulin (IgG, IgM) and complement (C3, C4) levels in Nigerian patients with nephrotic syndrome.
    • To compare these levels between patients with active disease and those in remission, and with healthy controls.
    • To explore the role of immunological defects in nephrotic syndrome.

    Main Methods:

    • Serum samples were collected from 31 adult Nigerian patients with nephrotic syndrome (28 active, 3 remission) and 11 healthy controls.
    • Concentrations of IgG, IgM, C3, and C4 were measured.
    • Statistical analysis was performed to compare groups.

    Main Results:

    • Patients with active nephrotic syndrome had significantly lower mean serum IgG levels compared to controls.
    • Mean serum IgM levels were significantly higher in patients with active disease than in controls.
    • Mean serum C3 and C4 levels were significantly elevated (P < 0.05) in active disease patients compared to controls.

    Conclusions:

    • The observed alterations in IgG, IgM, and complement levels in Nigerian patients with nephrotic syndrome mirror findings in Caucasian populations.
    • These results reinforce the hypothesis of a fundamental immunological defect contributing to the pathogenesis of nephrotic syndrome.
    • Further research into immune dysregulation is warranted for therapeutic strategies.
