    Background:

    • Nonfunctioning islet cell carcinoma is a rare pancreatic neuroendocrine tumor.
    • Accurate diagnosis relies on characteristic imaging findings.

    Purpose of the Study:

    • To define the computed tomographic (CT) features of nonfunctioning islet cell carcinoma of the pancreas.
    • To compare CT findings with ordinary pancreatic ductal adenocarcinomas.

    Main Methods:

    • Retrospective review of CT scans from 27 patients with nonfunctioning islet cell carcinoma.
    • Comparison of CT characteristics with 100 cases of ductal pancreatic adenocarcinoma.

    Main Results:

    • Pancreatic masses were identified in 96% of cases, with sizes ranging from 3-24 cm.
    • Calcification was present in 22% of tumors; 75% showed contrast enhancement.
    • Hepatic metastases (56%) and regional lymphadenopathy (37%) were common.
    • Features more characteristic of islet cell carcinoma included large size, calcification, and contrast enhancement.

    Conclusions:

    • CT imaging can identify characteristic features of nonfunctioning islet cell carcinoma.
    • Distinguishing islet cell carcinoma from ductal adenocarcinoma is possible based on size, calcification, and enhancement patterns.
