    A rare form of cholangiocarcinoma associated with liver cysts was diagnosed after initial suspicion of amebic abscess. This challenging diagnosis highlights the need for careful evaluation of liver cysts, especially when presenting with unusual symptoms.

    Background:

    • Cholangiocarcinoma, a cancer of the bile ducts, can present atypically.
    • Developmental liver cysts can be associated with malignant transformation.

    Observation:

    • A 46-year-old woman presented with right hypochondrial pain and liver cysts on ultrasonography.
    • Initial suspicion of amebic abscess based on cyst fluid was not confirmed.
    • Laparotomy revealed cholangiocarcinoma invading a nonepithelialized liver cyst.

    Findings:

    • The resected specimen showed cholangiocarcinoma associated with both epithelialized and nonepithelialized liver cysts.
    • Preoperative differentiation between benign cysts and malignant tumors was difficult.
    • This presentation represents a distinct entity from cystadenocarcinoma.
