Subendocardial infarction in two young boys.

D W Cotton, P J Gallagher, R A Sleet

    Summary

Subendocardial infarction can be observed in children following cardiac arrest. Assisted ventilation allows these myocardial changes to become visible at autopsy, impacting resuscitation survivor care.

    Area of Science:

    • Cardiovascular Pathology
    • Pediatric Critical Care

    Background:

    • Subendocardial infarction is a type of heart muscle damage.
    • Cardiac arrest in children presents unique challenges in resuscitation and post-arrest care.

    Observation:

    • Two cases of subendocardial infarction were identified post-mortem in young boys.
    • Both children experienced prolonged cardiac arrest and were supported with mechanical ventilation.

    Findings:

    • The myocardial damage (infarction) likely occurred during the cardiac arrest due to insufficient blood flow.
    • Extended mechanical ventilation enabled the gross and microscopic changes of infarction to develop and be detected.

    Implications:

    • This finding highlights the potential for developing myocardial infarction in pediatric cardiac arrest survivors.
    • Understanding this phenomenon is crucial for managing and assessing outcomes in children resuscitated from cardiac arrest.
