Primary hyperparathyroidism: changes in the pattern of clinical presentation.

G R Mundy, D H Cove, R Fisken

    Lancet (London, England)
    |June 21, 1980

    Summary

Primary hyperparathyroidism is increasingly detected through routine serum calcium screening. Women over 70 are most at risk, with unexpected hypercalcemia often being the first sign.

    Area of Science:

    • Endocrinology
    • Public Health

    Background:

    • Primary hyperparathyroidism (PHPT) is a common endocrine disorder.
    • Its incidence and presentation patterns are crucial for public health planning.

    Purpose of the Study:

    • To determine the current incidence and modes of presentation of primary hyperparathyroidism.
    • To identify patient demographics at higher risk.

    Main Methods:

    • Retrospective analysis of 207 hypercalcemic patients identified in an urban population.
    • Diagnosis of primary hyperparathyroidism based on established biochemical criteria.

    Main Results:

    • 111 cases of primary hyperparathyroidism were diagnosed.
    • Women over 70 represented 55% of diagnosed patients.
    • 57% of cases were incidentally discovered via serum calcium screening; 14% presented with acute hypercalcemic syndrome.

    Conclusions:

    • Routine serum calcium screening significantly contributes to early PHPT detection.
    • An estimated 250 new cases per million population per year are expected with modern diagnostics.
