    Background:

    • Rheumatoid arthritis (RA) is a chronic autoimmune disease primarily affecting joints.
    • Eosinophilic infiltration and vasculitis are rare but serious complications.

    Observation:

    • An elderly female RA patient exhibited malnutrition, elevated erythrocyte sedimentation rate, and peripheral eosinophilia.
    • Eosinophilic infiltration occurred in the pleura, uterus, skin, and muscle without granuloma formation.

    Findings:

    • The patient developed necrotizing vasculitis of the mesenteric vessels.
    • The vasculitis occurred without granuloma formation, a key diagnostic feature.

    Implications:

    • This case highlights that polyarteritis nodosa (PAN), including overlap types, can manifest as severe, widespread mesenteric vasculitis.
    • Early recognition and management of vasculitic complications in RA are crucial for patient outcomes.
