    Summary

    Laparoscopy revealed a unique "arterial coat" liver capsule thickening in patients with Polyarteritis Nodosa (PAN). This finding, absent in controls, may indicate segmental arterial lesions in PAN diagnosis.

    Background:

    • Polyarteritis Nodosa (PAN) is a systemic vasculitis affecting medium-sized arteries.
    • Laparoscopic diagnosis of PAN, particularly its hepatic manifestations, remains challenging.
    • Previous studies have not extensively detailed laparoscopic findings specific to hepatic involvement in PAN.

    Purpose of the Study:

    • To describe and analyze the laparoscopic findings in patients with histologically confirmed Polyarteritis Nodosa (PAN).
    • To identify potential unique laparoscopic signs indicative of hepatic involvement in PAN.
    • To compare laparoscopic findings in PAN patients with a control group.

    Main Methods:

    • Laparoscopic examination of 6 patients with biopsy-proven Polyarteritis Nodosa (PAN).
    • Detailed observation of liver capsule morphology during laparoscopy.
    • Histopathological analysis of biopsied liver capsule segments.
    • Comparison with 248 control laparoscopic examinations.

    • Five out of six PAN patients exhibited fibrous thickening of the liver capsule.
    • This thickening appeared as an "arterial coat" along arterial pathways with segmental distribution.

    • No similar findings were observed in 248 control laparoscopic examinations.

    Conclusions:

    • The observed "arterial coat" appearance of liver capsule thickening may represent segmental arterial lesions in PAN.
    • Laparoscopic findings of segmental capsular thickening could be a potential diagnostic marker for PAN.
    • Further research is required to determine the diagnostic sensitivity and specificity of these laparoscopic findings for PAN.
