    Background:

    • The vaginal microbiota plays a crucial role in maintaining urogenital health.
    • Lactobacilli are dominant bacteria in the healthy vaginal flora.
    • Understanding the presence of vaginal bacteria in urine is important for diagnosing urinary tract infections.

    Purpose of the Study:

    • To investigate the prevalence and significance of lactobacilli and other fastidious bacteria in mid-stream urine (MSU) samples.
    • To determine if these bacteria represent normal flora or potential contaminants in healthy women and those with urinary symptoms.

    Main Methods:

    • Urine culture of MSU samples from 142 healthy women and 26 women with dysuria/frequency.
    • Identification of lactobacilli and other fastidious bacteria (anaerobes, CO2-requiring).
    • Quantification of bacterial counts and comparison between groups.

    Main Results:

    • Lactobacilli were frequently isolated from healthy women's urine (≥10^4/ml in 34.8%, ≥10^5/ml in 20.2%).
    • Anaerobic gram-positive cocci were also commonly found, consistent with vaginal flora.
    • Similar bacterial flora and counts were observed in women with urinary symptoms but no conventional pathogens.

    Conclusions:

    • The presence of lactobacilli and vaginal commensals in urine samples is common and likely represents contamination or normal flora.
    • These findings suggest that lactobacilli in urine, in the absence of pyuria or conventional pathogens, do not necessarily indicate a urinary tract infection.
