Neuroendocrine differentiation in basal cell carcinomas.

    Summary

    Basal cell carcinomas can show neuroendocrine differentiation, a finding supported by immunohistochemical analysis of hormone markers. This suggests a potential for multidirectional differentiation in neoplastic epithelial cells.

    Background:

    • Basal cell carcinoma (BCC) is the most common human cancer.
    • Previous studies suggested neuroendocrine differentiation in BCC via Grimelius stain and electron microscopy.

    Purpose of the Study:

    • To investigate neuroendocrine differentiation in basal cell carcinomas using immunohistochemistry.
    • To identify specific neuroendocrine markers present in BCC.

    Main Methods:

    • Immunohistochemistry was performed on ten BCC cases.
    • Tissue localization of calcitonin, insulin, glucagon, somatostatin, ACTH, gastrin, and nerve growth factor was assessed.
    • Staining specificity was confirmed by antigen absorption.

    Main Results:

    • Nine BCCs showed typical histology; one exhibited features resembling oat cell carcinoma.
    • Three cases were negative for all tested hormones.
    • Focal positivity for single or multiple hormones (calcitonin, somatostatin, ACTH) was observed in seven cases.
    • Positive staining was abolished by specific antigen absorption.

    Conclusions:

    • Basal cell carcinoma can exhibit neuroendocrine differentiation.
    • This supports the concept of multidirectional differentiation potential in neoplastic epithelial cells.
    • Immunohistochemistry is valuable for detecting neuroendocrine features in BCC.
