Drug Abuse and Addiction: Pharmacological Phenomena01:15

Drug Abuse and Addiction: Pharmacological Phenomena

1.5K
Drug dependence, abuse, and addiction are complex phenomena that can precipitate various abnormal states. Physical dependence refers to a state of pharmacological adaptation to a drug. This adaptation often results in tolerance—a reduced response to the drug after repeated administrations. When the drug use is abruptly stopped, withdrawal symptoms occur due to the body's need to readjust from the pharmacologically induced imbalance. However, tolerance and withdrawal symptoms do not...

Opioid Analgesics: Synthetic and Semisynthetic Opioids

1.5K
Synthetic and semisynthetic opioids are pivotal in pain management and tackling opioid addiction. Semisynthetic opioids, including morphinans (morphine derivatives), oxycodone, oxymorphone, hydrocodone, and hydromorphone, have improved pharmacokinetic profiles compared to morphine. Additionally, heroin and 6-MAM (6-Monoacetylmorphine) show better CNS penetration than morphine due to heightened lipid solubility. Hydromorphone, a potent opioid, undergoes hepatic metabolism to form the active...

Phase II Reactions: Methylation Reactions01:17

Phase II Reactions: Methylation Reactions

881
Methylation is a phase II biotransformation process involving the attachment of a methyl group to a substrate. Enzymes known as methyltransferases orchestrate this reaction.
The mechanism of methylation unfolds in two stages. The first stage sees a methyltransferase enzyme facilitating the transfer of a methyl group from S-adenosylmethionine (SAM) to the substrate, forming S-adenosylhomocysteine (SAH). The second stage involves further metabolism of SAH into homocysteine, which can be recycled...

Therapeutic Drug Monitoring: Overview and Classification

523
Therapeutic Drug Monitoring (TDM) is a clinical practice that measures specific drug levels in a patient's blood at designated intervals to ensure the drug concentration stays within a therapeutic range. This monitoring is crucial for optimizing individual dosage regimens, enhancing therapeutic efficacy, and minimizing drug-related toxicity. TDM is vital for drugs with narrow therapeutic windows, significant variability in pharmacokinetics, and a clear correlation between plasma levels and...

Therapeutic Drug Monitoring: Affecting Factors

335
Therapeutic Drug Monitoring (TDM) is the clinical practice of measuring specific drug levels in a patient's blood or body tissues to manage and optimize therapy. TDM is crucial for drugs with narrow therapeutic windows, like warfarin and phenytoin, where incorrect doses can lead to treatment failure or severe side effects. This monitoring ensures the dosage administered is within a safe and effective range. The factors affecting therapeutic drug monitoring include:Patient-Specific Factors:a.

Therapeutic Drug Monitoring: Drug Analysis Methods

296
Therapeutic Drug Monitoring (TDM) is a clinical practice that measures specific drug levels in a patient's blood or body tissues to tailor drug therapy effectively. This monitoring is critical for managing drugs with narrow therapeutic indices like digoxin and phenytoin, ensuring they are both safe and effective. For instance, monitoring theophylline levels in asthma patients involves precision and sensitivity to adjust doses according to individual responses to therapy, ensuring efficacy and...

    Summary

Former heroin users had more speeding tickets after starting methadone maintenance. Driving records, excluding speeding, did not significantly change, suggesting methadone patients can retain driving privileges.

    Area of Science:

    • Addiction medicine
    • Forensic psychology
    • Traffic safety research

    Background:

    • Opioid use disorder (OUD) treatment often involves methadone maintenance therapy (MMT).
    • Driving impairment and traffic safety are critical concerns for individuals with OUD.
    • Previous research on driving performance during MMT is limited and sometimes contradictory.

    Purpose of the Study:

    • To compare the motor vehicle driving records of former heroin users before and after initiating methadone maintenance.
    • To assess changes in traffic violations and accident involvement associated with MMT.
    • To inform policy regarding driving privileges for patients undergoing methadone treatment.

    Main Methods:

    • Retrospective analysis of driving records for 104 former heroin users.

  • Comparison of driving records during a 1-year period of heroin use versus a 1-year period of methadone maintenance.
  • Statistical analysis of convictions for speeding, negligent collisions, other moving violations, driving without a license, and accidents.
  • Main Results:

    • A statistically significant increase in speeding convictions was observed after admission to methadone maintenance compared to the period of heroin use.
    • No significant changes were found in convictions for negligent collisions, other moving violations, driving without a license, or accidents.
    • Accident involvement rates did not differ significantly from the general Texas licensed driver population.

    Conclusions:

    • Heroin users may exhibit slightly better driving records while on heroin, potentially due to increased caution to avoid arrest.
    • Methadone maintenance does not appear to significantly worsen overall driving safety, with the exception of increased speeding.
    • The study recommends against restricting driving privileges for individuals maintained on methadone.
