    Area of Science:

    • Dermatology
    • Immunology
    • Allergy

    Background:

    • Delayed pressure urticaria (DPU) is an under-recognized dermatological condition.
    • Characterized by delayed onset of urticarial lesions following pressure application.

    Purpose of the Study:

    • To describe the clinical features, diagnostic methods, and treatment outcomes of 17 patients with DPU.
    • To investigate potential underlying mechanisms and disease course.

    Main Methods:

    • DPU was induced using a standardized pressure challenge (15 pounds for 15 minutes).
    • Clinical presentation, laboratory findings, skin biopsies, and response to various medications were assessed.
    • Disease course and treatment requirements were categorized.

    Main Results:

    Related Experiment Videos

    • DPU lesions appeared after an average of 6.5 hours, were painful, peaked at 9 hours, and resolved within 24-48 hours.
    • Systemic symptoms (fever, chills, arthralgias) occurred in 78% of patients.
    • Antihistamines were effective for urticaria, NSAIDs for pressure lesions, and prednisone was required in 87.5% of patients.

    Conclusions:

    • DPU is more prevalent than previously thought and may involve mediators beyond histamine, potentially including the prostaglandin system.
    • Effective management often requires a combination of antihistamines, NSAIDs, and corticosteroids.
    • DPU exhibits variable disease courses, ranging from mild to severe and nonremitting.
